Gamefaqs' scan is of the CERO D variant?  ??? I would say that there are two different versions, but I'm not 100%: it could be a fake image.

A Cero D release would mean the game came out 2006 or later. It could have been one of those thing where a game was rated but not released. We won't know until we can find a picture of one.

This is a case of having more information than the database current can account for. I do not think it is (currently) a good idea to add this kind of variation. Rather, you can put in the description that the actual discs may be of x types, and then when you add the item into your collection, use the Notes portion to put which one you have.

This is my opinion.

Sony uses the terms 'slim' and 'slimline' for many of their products. It is a description of a design or a redesign. No where can I find it being used as a product name. Other things they use slim with are VAIO notebooks and televisions.

No it is Crash Bandicoot: Warped. The 3 on the artwork is because there is a dial behind the logo. There is a 1, 2, 3 and 7 (at least) visible behind it. When in doubt concerning a game's title, try to find it used elsewhere on the packaging, primarily the spine or on the back or manual. On this particular example, you can find the title on the back in the copyright information. "Crash Bandicoot: Warped TM & (c) 1998 Universal Interactive Studios Inc..."

Furthermore, many movies are remakes with the same title. If the year is the arbitrary UMD date, how do we distinguish which movie it is, other than resorting to adding the year into the title?

What I'm getting at is that shape designations are redundant in a title entry where the model number already exists. There would be no time ever that a single model number could be any of the different shapes the console itself may have.

ALSO: the title will need to change at some point, but more research is required to determine which edition is which.
