It can be hard to search for some of these games depending on style guide adherence.  Since the search is strict, if you don't get it exactly right, you'll miss what you're searching for.

Fixed and repurposed all except for D for PS1. Was this only released as long box? If it was I will merge the two.
Also as darko pointed out, was http://vgcollect.com/item/19310 (Disney Learning Preschool) released? it shows that 1 person has the game.

No, there was the longbox (plastic jewelcase) release, and there was a double jewel case release. See here.

I've been hunting for the double jewelcase release for years.
