The second listing does offer alternate boxart, but with no other information noted, I'm not sure if it's something that needs a separate listing.

The alternate artwork was a custom that people have made to be inserted into plastic cases. From what I can tell, this cart was included with a system on its own and was not released in a box at all.

Yeah that might be the best course of action. I didn't know what quite to do with it since there will be no "boxed" version that includes the watch. The watch (item record to be made still) will be swag, similar to the watches from the other VLR games.

Use the extra entry (the watch) to make the swag entry.
Please confirm that packaging first. If it hasn't been released yet, it shouldn't be in the db as being released.
