Games bundled with graphics card - what to use for box art?

I have a few loose games which I picked up in a file case I bought at a flea market, and they are most likely from an ASUS EAX1600XT card bundle. Previously I just entered them individually, but what would be the best approach here? A single entry? Or just leave them separate?

What's the best thing to use for box art? Or should I just leave this blank?

Looks good, but me personally If there's no box art, I like to use the disc itself as the main image. I think it makes your collection more appealing to look at overall.

I have a lot of loose discs that came bundled with old graphics cards, and usually I'll try to find a picture of the disc, or take a picture of the disc myself.  Looks like you did just that, very nice!  ;D
